
Lower Hudson County Care Coalition members
Peace Care St. Ann’s recently hosted Healthcare Quality Strategies, Inc. (HQSI), the Medicare quality improvement organization for New Jersey, and the members of its Lower Hudson County Care Coalition.
The Coalition’s project is an effort to improve coordination of care for Medicare patients with a goal of reducing avoidable hospitalizations, readmissions, and adverse drug events. By bringing together healthcare providers, social service providers, and other stakeholders, situations can be identified in which coordinating efforts across organizations can improve the care of Medicare beneficiaries. These situations would involve those individuals who are at high risk of being admitted or readmitted to the hospital, those with multiple chronic health conditions or who lack support at home.
